Stephanie Thomopoulos, Care Manager/Clinical Social Worker and owner of GeriatriCare® Management, Inc. is collaborating with Dr. Simon Fishman, Neurologist with Integrated Neurology Services to provide coordinated care for mutual clients. This coordinated care service is available to clients that have Neurocognitive Disorder (Alzheimer’s Disease/memory impairment), traumatic brain injury, physical limitations due to chronic neurologic illness (Parkinson’s Disease, Multiple Sclerosis, etc.), or repeated falls that live in their home or in long term care facilities in Northern Virginia.
Clients requiring assistance are evaluated by both practitioners independently. While Dr. Fishman completes a thorough medical/cognitive evaluation, Stephanie’s evaluation takes place at the home or in the long term care facility of the client. Her evaluation includes a thorough review of the client’s overall situation, including evaluations of: psycho social issues, care giving issues, community resources, placement considerations, aging in place, eligibility for government/financial assistance, home safety evaluation and ongoing care coordination. Through the use of TeleMedicine, this collaboration provides expedited on-sight clinical reviews, which can help resolve most issues, such as any change of condition. This team approach helps manage difficult situations which could otherwise lead to a physical/cognitive decline or hospital admission/readmission.
