GeriatriCare® Management, Inc. was founded in 1996 by Stephanie Thomopoulos, a Licensed Clinical Social Worker, with over 30 years of experience working with the elderly, the disabled and their families. As a Professional Care Management agency, we understand and can assist with many of the common problems affecting this population, such as memory loss, relocation considerations, grief, and conflicts concerning care issues. We are dedicated to providing the highest quality of client care with compassion and respect for each person. Our mission is to provide the most complete range of services available to meet the needs of our clients. We use a team approach and an extensive referral system to enable clients to choose from multiple options that are personalized to individual or family needs.
A licensed Home Care Agency was added in 2006 to ensure that there was continuity in all aspects of our client’s health care needs. GeriatriCare® Home Health provides a broad range of non-medical, supportive services ranging from meal preparation and light housekeeping to taking a client to doctor appointments and assisting with bathing and dressing. To better serve or clients, in May 2017 the Home Care Agency was merged with a national company, Home Care Assistance. We continue to work closely with Home Care Assistance and other agency’s to coordinate services to serve our mutual clients.
Our commitment is to support the client with appropriate services in a cost-effective manner recognizing that most people when possible want to be supported in their own home. Other programs offered by GeriatriCare® Management, Inc. include Coordination of Services, Medication Management, the Friendly Visitor Program, Plan Now/ Care Later, Guardianship and Conservatorship, and Pet Therapy.
